Integrated Services for Seniors

The Integrated Services for Seniors (ISFS) program improves access to services for older adults aged 55+ in Thorncliffe Park, Flemingdon Park and surrounding areas,  by providing support to necessary Primary Care, Allied Care and community services. This program focuses on active living, enabling older adults to age in place, remain connected to health care and support services through integrated service delivery, and keeping seniors connected to appropriate service providers. The ISFS Team supports seniors with emerging concerns, monitoring their development, mobilize needed resources, support independence, encourage participation throughout the community and also in congregate settings through a variety of scheduled events.  This program empowers seniors to connect to resources and offer collaborative and inclusive programming that maintains and promotes the physical, emotional and cognitive well-being of the clients served. This program is inclusive of intake & referral navigation, program facilitation, outreach, casework and volunterism.

Personal Support Services for Low Acuity Home Care Seniors

The Personal Support Services (PSS) for Low Acuity Home Care program will assist older adults aged 55+ in Thorncliffe Park, Flemingdon Park and surrounding areas, this includes assisting seniors by mobilizing the necessary Primary Care, Allied Care and Community Care supports through its team of Community Support Liaisons (CSL). This program supports and enables seniors to age at home, remain connected to health care and support services through integrated service delivery, ensuring seniors are connected to appropriate service providers. This program will also coordinate the provision of travel, interpretation, respite services, primary care, companionship, social/leisure activities and other community health services within our inter-agency and local partner frameworks.

The PSS Team will use individualized person-centered approaches to client care, this includes supporting activities of daily living (ADL’s), from personal care/hygiene, limited housekeeping (no cleaning), providing reminders, supervision, escorting to appointments/errands, support or companionship related to overall health/wellbeing, leisure, education, fitness, wellness and social activities according to established individualized care plans.
