Health Access Thorncliffe Park connects Thorncliffe Park residents with essential health and wellness services. Offering multilingual support, it provides easy access to family doctors, mental health resources, dental care, and more. Your one-stop for community-focused healthcare!

Health Access Thorncliffe Park (HATP) is community health clinic that provides health and wellness services and connects people to other services. We offer primary health care, Social Work, nutrition, family support, Health Education and Promotion, Footcare services and a pediatric clinic.

 

Our team talks with you about all your needs. We explore your physical health; your emotional and social well-being and we connect you to services or supports you may need.

 

HATP is now providing the following services:

  • Basic Primary Care and Family Medicine – Blood pressure management, cancer screening, women’s health, and diabetes services
  • Social Work – Stress management and mental wellness counselling
  • Nutrition – Support to adults, families and children to meet nutrition needs, including weight management, picky eating and high cholesterol
  • Family supports – Including healthy child development
  • Health education – Individual and group sessions available
  • Health promotion – Clinical groups that help address the social determinants of health
  • Chiropody – Basic foot care services to seniors & people with visual impairments, diabetes, mental health concerns, and homelessness
  • Community Resources and Referrals – making the right connections and referrals to services for both clients and service providers
  • Child and Youth Psychiatry and Adult Psychiatry
  • Psychological services for children including psycho-educational assessments, behavioural consults and individual psychotherapy
  • General Pediatricians and Developmental Pediatricians
  • Prenatal Care

How to join/Register (Please provide the process map including, if they are new clients, what’s the intake process, what if existing clients, etc. Email ID to be linked and all necessary information. We want to make this process simple, seamless and online. New clients will be connected to the intake counsellors for registration. The intake counsellors will connect you to any of our internal services or navigate you to external services. Returning clients will be directed to the right service

Who is Eligible?

Our services are free and open to all* residents living in postal code M4H. Our services can be accessed without a health card and without a medical referral. Language interpretation services are available as needed.

HATP services are offered for clients who:

  • Don’t have a Health Card (OHIP) and are in the 3-month waiting period
  • Do not have legal immigration status (non-status)
  • Are in the refugee claim process and are un-insured
  • Have coverage under (IFHP) Interim Federal Health Program
  • Have a health card but not attached to a family doctor
  • Has a family doctor but needs to see Dietitian or a Social Worker

*Visitor visa and work permit holders as well as international students are not eligible
