
Personal Support Services for Low‑Acuity Home Care Seniors
Program Overview
TNO delivers a continuum of home-based supports, including:
Personal Support Services
Assisted Living Supports
Enhanced Respite Care
Together, these services enable seniors to remain in their homes while ensuring timely access to primary care, allied health, and community-based supports. Services are delivered by a dedicated team of Personal Support Workers (PSWs) and Community Support Liaisons (CSLs) who work collaboratively with inter-agency and local partners.
Through care coordination and service navigation, the program ensures seniors are connected to the appropriate level of care at the right time, supporting continuity, safety, and quality of life.
Person-Centred and Integrated Care Approach
Services are guided by individualized, person-centred care plans that reflect each client’s goals, preferences, and cultural context. The program supports both Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) while addressing broader social determinants of health.
Supports may include:
In-home personal care supports
Companionship and social engagement
Medication reminders (non-clinical)
Caregiver respite
Light household supports (no cleaning services)
Coordination with healthcare and community service providers

Focused on physical assistance and daily living supports, including:
Assistance with bathing, dressing, grooming, toileting, and mobility
Meal preparation support
Support to maintain routines and independence

Health and Wellness Monitoring
Medication reminders
Observation and reporting of changes in condition
Coordination and communication with primary care and health partners

Safety and Supervision
Regular wellness checks
Fall prevention support
Monitoring to support safe aging at home and during emergencies

Social and Emotional Support
Companionship and conversation
Support with social and leisure activities
Meaningful engagement to reduce isolation and loneliness

Household Support
Light household tidying
Limited laundry support
Assistance maintaining a safe and organized home environment (No cleaning services provided)
Enhanced Respite Care
Provides temporary, short-term in-home support to ensure continuity of care while primary caregivers attend to work, rest, or personal needs. Respite services support caregiver well-being while maintaining stability for the client.
Coordinated Community Partnerships
The program operates within strong inter-agency and local partner frameworks, coordinating access to:
Primary and allied care
Interpretation and language supports
Transportation and travel assistance
Community health services
Recreational, social, and leisure activities
This holistic and integrated service delivery model strengthens connections between home care, community supports, and healthcare services—supporting seniors to live safely, independently, and with dignity in their communities.

Our Services are open to older Adults 55+, living in Thorncliffe, Flemingdon, St James Town & Teesdale

Hours & Locations
Monday – Friday 9:00 AM – 5:00 PM (Enhanced Respite Care expended hours evening/nights/weekends)
Location: Thorncliffe Park, Flemingdon Park, St. James Town, Teesdale, and surrounding areas
