The Integrated Services for Seniors program supports adults aged 55+ living in Thorncliffe Park, Flemingdon Park, and surrounding communities. The program promotes active, healthy aging by strengthening community connections and supporting seniors to remain safely, comfortably, and confidently in their homes and neighbourhoods.

The program focuses on belonging, independence, and meaningful engagement, helping seniors stay connected to their communities, peers, and supports that enhance overall well-being.

Services for Seniors 55+ in Thorncliffe and Flemingdon Park

Our team works closely with seniors experiencing emerging or changing needs, offering early support, ongoing monitoring, and connection to appropriate services. We prioritize maintaining independence, reducing isolation, and encouraging participation in both community and congregate settings.

Seniors are engaged through a variety of scheduled physical, social, educational, and recreational activities, including:

  • Group-based wellness, fitness, and social programs

  • Educational workshops and skill-building opportunities

  • Seasonal and cultural celebrations

  • Congregate and home-delivered meal programs

Programs and Services

The Integrated Services for Seniors program delivers coordinated, community-based supports that promote independence, social connection, and well-being. Services are person-centred, culturally responsive, and equity-informed, supporting seniors to age safely and confidently in their communities.

Programs and services include:

  1. Service Arrangement & Care Coordination
    Provides intake, assessment, outreach, and service planning, connecting seniors to appropriate community supports based on their needs and goals. This includes coordination of social, recreational, wellness, fitness, and physical activity programs, as well as navigation to other community and health-related services. Care coordination supports timely access, continuity, and reduced service fragmentation.

  2. Case Management – Coordinated Care
    Offers individualized case management and care coordination for seniors with emerging or complex needs. Services include ongoing assessment, goal setting, monitoring, and collaboration with primary care, healthcare providers, and community partners to support integrated, wraparound care.

  3. Visiting – Social and Safety Supports
    Provides social visiting, wellness checks, and safety monitoring for seniors who may be socially isolated or at risk. These supports enhance social connection, provide reassurance, and help identify changes in health or social needs to support aging safely at home.

  4. Social and Congregate Dining
    Offers congregate dining and meal programs that support nutrition, social participation, and community belonging. These programs create inclusive spaces for seniors to connect, reduce isolation, and engage in meaningful social interaction.
