
Integrated Services for Seniors
Holistic, Community-Centred Support
The program supports seniors’ physical, emotional, and cognitive well-being through inclusive and collaborative services that include:
Intake, assessment, and referral navigation
Outreach and relationship-based engagement
Individual and group casework
Program planning and facilitation
Volunteer opportunities that strengthen peer support and community involvement
Seniors are supported as active participants in their own care and community life, with services tailored to their interests, strengths, and goals.
Strengthening Community Connections
At the core of the program is a commitment to positive community support and connection. Seniors are encouraged to participate in meaningful activities, develop new skills, and build strong social networks that enhance quality of life and reduce social isolation.
Where appropriate, the program integrates a social prescribing approach to connect seniors with local, non-clinical community programs and services that complement health and social supports. This helps bridge community and healthcare services while keeping the focus on relationships, engagement, and community belonging.
Through strong partnerships and a person-centred approach, the Integrated Services for Seniors program supports aging with dignity, independence, and connection—helping seniors feel supported, empowered, and at home in their communities.
Programs and Services
The Integrated Services for Seniors program delivers coordinated, community-based supports that promote independence, social connection, and well-being. Services are person-centred, culturally responsive, and equity-informed, supporting seniors to age safely and confidently in their communities.
Programs and services include:
Service Arrangement & Care Coordination
Provides intake, assessment, outreach, and service planning, connecting seniors to appropriate community supports based on their needs and goals. This includes coordination of social, recreational, wellness, fitness, and physical activity programs, as well as navigation to other community and health-related services. Care coordination supports timely access, continuity, and reduced service fragmentation.Case Management – Coordinated Care
Offers individualized case management and care coordination for seniors with emerging or complex needs. Services include ongoing assessment, goal setting, monitoring, and collaboration with primary care, healthcare providers, and community partners to support integrated, wraparound care.Visiting – Social and Safety Supports
Provides social visiting, wellness checks, and safety monitoring for seniors who may be socially isolated or at risk. These supports enhance social connection, provide reassurance, and help identify changes in health or social needs to support aging safely at home.Social and Congregate Dining
Offers congregate dining and meal programs that support nutrition, social participation, and community belonging. These programs create inclusive spaces for seniors to connect, reduce isolation, and engage in meaningful social interaction.

Our Services are open to Older Adults 55+, living in Thorncliffe & Flemingdon

Monday – Friday 8:00 AM – 5:00 PM
Location: Thorncliffe Park, Flemingdon Park, and surrounding areas
