Supporting older adults to remain independent at home through invidualized care plans for up to 12 weeks, with eligible extensions available based on individual needs.

Who is Eligible?

Our Services are open to older Adults 55+, living in Thorncliffe Park, Flemingdon Park, St. James Town, Teesdale, Taylor Massey and surrounding areas.
