Health Access Thorncliffe Park (HATP) - Co-led by TNO
Health Access Thorncliffe Park (HATP) is a community‑based health and wellness initiative co‑led by The Neighbourhood Organization (TNO) and Flemingdon Health Centre (FHC). HATP provides free, accessible, and comprehensive primary health care services to residents of Thorncliffe Park, delivered through a model that centres community trust, equity, and holistic care.
At TNO, we believe health is shaped not only by medical needs, but also by social, emotional, cultural, and economic realities. HATP reflects this belief by embedding health services within a trusted community hub and integrating clinical care with holistic intake, navigation, and community‑based support.

The Corner is a collaborative community hub in St. James Town designed to provide accessible health, social, and community services under one roof. Operating across multiple locations, it offers a wide range of programs including one-on-one support, group activities, and essential services such as internet access and community resources. Powered by partnerships with local organizations and active resident involvement, The Corner responds to the diverse and evolving needs of one of Toronto’s most densely populated neighbourhoods, fostering inclusion, well-being, and community engagement.
