Health Access Thorncliffe Park (HATP) is a community‑based health and wellness initiative co‑led by The Neighbourhood Organization (TNO) and Flemingdon Health Centre (FHC). HATP provides free, accessible, and comprehensive primary health care services to residents of Thorncliffe Park, delivered through a model that centres community trust, equity, and holistic care.  

At TNO, we believe health is shaped not only by medical needs, but also by social, emotional, cultural, and economic realities. HATP reflects this belief by embedding health services within a trusted community hub and integrating clinical care with holistic intake, navigation, and community‑based support.

At HATP, residents are supported as a whole. Our interdisciplinary team takes time to understand each person’s physical health, mental and emotional well‑being, family situation, settlement experience, and access to resources. Care is coordinated so that residents receive the right support, at the right time, in ways that are respectful and responsive.

Holistic Intake and Navigation Counsellors (HINCs) 

Holistic Intake and Navigation Counsellors (HINCs) 

A defining feature of the HATP model is the role of Holistic Intake and Navigation Counsellors (HINCs). Rooted in TNO’s community‑embedded practice, HINCs are often the first point of contact for residents’ accessing services. 

HINCs provide comprehensive, strength-based intake and navigation support, taking time to understand the full context of a person’s life—not just their immediate health concern. They work closely with clinical providers and community partners to:

HINCs provide comprehensive, strength-based intake and navigation support, taking time to understand the full context of a person’s life—not just their immediate health concern. They work closely with clinical providers and community partners to: 

  • Conduct holistic assessments that consider health, mental well‑being, settlement, income, housing, and family needs 

  • Reduce barriers related to language, system complexity, documentation, or past experiences of exclusion 

  • Support residents to navigate health care, mental health services, and social supports 

  • Ensure continuity of care through warm referrals and follow‑up 

By bridging health care and community services, HINCs play a critical role in preventing residents from falling through gaps and ensuring care is coordinated, dignified, and accessible.

Community Health Workers and Health Promoters 

Community Health Workers and Health Promoters 

Community Health Workers (CHWs) and Health Promoters are essential to how HATP connects with residents and responds to community priorities. Grounded in TNO’s community development approach, these roles are built on trust, lived experience, and strong relationships within Thorncliffe Park.

  • Share culturally and linguistically appropriate health information 

  • Support residents in accessing HATP services and understanding care plans
